Please be advised, effective 8/1/2014, the Named Driver Exclusion Authorization Form FM-1001 will be available on-line through iBIS printable documents.   It will continue to be necessary to submit this form, including the insured’s signature, prior to issuance. 

Once issued, this form will be included in the declaration along with the Named Driver Exclusion Endorsement Form – 00510.  Minor changes have been made to this Endorsement for added clarity and consistency.
