For each Patriot Life Submitted Application between June 1^st and August 31^st, the writing producer will be entered into a drawing for a $500 Meijer Gift Card.

For each submitted application between June 1^st and August 31^st and placed in force before Sept. 30^th, the writing producer will receive the following:

$10 Meijer Gift card for 1^st policy

$20 Meijer Gift Card for 2^nd policy

$30 Meijer Gift Card for 3^rd policy

$40 Meijer Gift Card for 4^th policy

$50 Meijer Gift Card for 5^th and every policy submitted and issued during the Contest Period
