Effective September 1, 2011, we are making a series of important adjustments to our personal lines forms.  Please review the following detailed descriptions of these changes: 

HOMEOWNERS

HO-0134, Special Provisions – – Ohio (03-06 to 09-11)

The “Agreement” section has been revised to indicate the application is incorporated, by reference, as a part of the policy. 

The “Additional Coverages” section has been revised to indicate Ordinance or Law Coverage, for HO-0004 policies, is included as a percent of the Building Additions and Alterations limit. 

FM-1631 (9-11), Notice to Policyholder, Special Provisions – Ohio, will be attached to renewals for a period of one year.

FM-434i, Homeowner Application (03-09 to 09-11)

A statement is added, before the Applicant’s Signature, to indicate the application is expressly incorporated by reference in any policy issued by the insurer to the applicant.

In addition, some questions/information has been added to the skeleton of the application (Agency Contact, Agency Contact E-Mail Address, Agency Requests E-Delivery, Proxy Clause Question, Premium Disclaimer).

 

BOAT-SPORT VEHICLE

97280, Amendatory Endorsement, Boat and Sports Vehicle Policy – – Ohio (03-06 to 09-11)

The “Agreement” section has been revised to indicate the application is incorporated, by reference, as a part of the policy.

FM-1649 (9-11), Notice to Policyholder, Amendatory Endorsement, Boat and Sports Vehicle Policy – – Ohio, will be attached to renewals for a period of one year.

 

PERSONAL AUTO

02560, Amendatory Endorsement, Personal Auto Policy (11-09 to 09-11)

The “Agreement” section has been revised to indicate the application is incorporated, by reference, as a part of the policy. 

“Part A, Liability Coverage Exclusions” has been revised to reference family member, rather than relative, as it is a defined term in the Policy. 

“Part C, Uninsured Motorists Coverage Limit of Liability” has been revised to note that payment will not be made for the same elements of loss under Part C, Uninsured or Underinsured Motorists and Part A, Liability, or Part B, Medical Payments.

“Part C, Uninsured Motorists Coverage Legal Action” has been revised to include Underinsured Motorists in the 3 year limitation applicable to Uninsured Motorists.

FM-1632 (9-11), Notice to Policyholder, Amendatory Endorsement, Personal Auto Vehicle Policy, will be attached to renewals for a period of one year.

 

FM-464i, Personal Automobile Application (06-09 to 09-11)
