The Application For Benefits (AFB) is a packet of forms likely to be sent out when a Personal Injury Protection (PIP) claim is presented. Returning the completed forms in a timely manner helps to ensure prompt and accurate processing of the claim. The AFB is necessary to process payment of bills submitted by providers. When someone is injured in a motor vehicle accident, whether they are a Medicare beneficiary or not, forms are required to be filed with Medicare regarding any injuries they sustained in the motor vehicle accident. Properly completing the applicant’s given legal name, social security number, and any injuries that may have been sustained enables us to properly complete the required forms with Medicare regarding the claim.
There are also sections to document out of pockets expenses and wage loss. If the adjuster is unaware of the expenses, they are unable to consider any reimbursements. By returning the forms, the reimbursements can be considered for payment.
