It is our goal to maintain updated and accurate information on all policyholders. To ensure this, some of your policyholders will begin seeing one of the following “Notice to Policyholders” forms attached to renewals effective November 1, 2014.  Contained in this “Notice to Policyholders”, we are asking insureds to review the roofing information displayed on their declaration and notify us of any changes.  This includes both the age and type of roof.  We are asking that you forward their updates to our attention at PL.Email@fmins.com.   

Michigan Homeowners:

FM-1807, Notice To Policyholders Homeowners Roofing Information 

This Notice will attach to Homeowner Renewal policies with the exception of Manufactured Homes, Tenant and Condo policies.

Ohio Homeowners:

FM-1807, Notice To Policyholders Homeowners Roofing Information 

This Notice will attach to Homeowner Renewal policies with the exception of Tenant and Condo policies.

Ohio Dwelling Fire:

FM-1808, Notice To Policyholders Dwelling Fire Roofing Information 

This Notice will attach to Dwelling Fire Renewal policies with the exception of Unit-Owners (Condo) policies.
